Description: Through connected care at home capabilities, we will explore how these technologies enable behavior change and empower individuals and clinicians to make informed decisions about healthcare treatment. By utilizing technology, healthcare providers can enhance patient engagement, allowing for personalized and proactive care management that drives better health outcomes and better care experiences. Additionally, this session will showcase how these technologies optimize resource utilization, streamline care delivery, reduce length of stays in the acute setting, reduce readmissions leading to a tangible return on investment for healthcare organizations. The session will feature a real-world case study, demonstrating the successful integration of digital health technologies into care delivery models.
