Title: Informatics-Driven Framework for Evaluating Health Systems of Care

Description: Establishing robust systems of care are crucial for ensuring care continuity across the spectrum of pre-hospital, in-hospital, and post-hospital settings. Utilizing data from a tertiary healthcare system comprised of a network of 7 certified stroke centers, we present our experience and preliminary findings from a research initiative in which we connect health information from across multiple data modalities. Our overall objective is to leverage mixed computational methods (e.g., sequential pattern mining, process mapping, recurrent neural networks) to develop a decision-support model to identify stroke survivors at increased risk of undergoing unfavorable health trajectories during their recovery and rehabilitation.
